INCIDENTS
89-314 - Grand Canyon (Arizona) - Air Crash with Injuries
A Cessna T-207 operated by Lake Powell Air Service, a commercial tour
company, crashed while making a forced landing on the Tonto Plateau ten
miles west of Phantom Ranch yesterday, injuring the pilot and his two
passengers. The pilot, R.B., 27, of Lancaster, California,
suffered a head injury and multiple broken bones, including his pelvis, jaw,
ribs and ankle, and was flown by helicopter to a hospital in Flagstaff.
Passengers K.D., 28, and A.R., 27, both of Germany,
were treated at the Grand Canyon Clinic for, respectively, a broken arm and
a broken leg and possible spinal injuries. The plane left Page around noon;
approximately 30 minutes later, R.B. called the control tower at Grand
Canyon Airport and reported that he had some type of unspecified problem
with the aircraft. A short time later, he attempted to land the plane,
which flipped onto its back after touching down. Although the crash
occurred outside the corridor where tour planes are required to fly, it is
thought that the plane may have strayed there because of its problems. (AP

89-315 - Grand Portage (Minnesota) - Missing Person
On October 4th, J.S., 36, and A.D., 50, both of Duluth,
were camping near Fort Charlotte when J.S. left the campsite to take some
photos. After several hours had passed and J.S. did not return,
A.D. began searching for him. He failed to find J.S., so hiked out
and reported the incident to park rangers. Rangers and Cook County Sheriffs
Office deputies began a search for J.S., but suspended their efforts on
October 8th after failing to find any trace of hm. It is believed that
J.S. may have slipped on some rocks and fallen into the Pigeon River.

1) Shell Oil Company's Martinez Manufacturing Complex nay have shipped some
inadequately odorized propane to retailers in northern California, southern
Oregon and western Nevada this spring and summer, and they have asked that
the following message be posted in prominent locations in camping facilities
in California, Oregon, Washington, Idaho, Nevada, Arizona, Utah, Montana,
Wyoming, Colorado, and New Mexico: "PROPANE SAFETY NOTICE: ATTENTION RV
OWNERS - If you purchased propane for your RV in Northern California, the
Bookings, Oregon, area, or the Reno-Lake Tahoe area between April 1 and
September 4, 1989, and have not refilled since that time, there is a
possibility that your propane does not contain the proper amount of odorant.
Because propane is odorless, a special ingredient is added to make it
detectable by smell. The 'rotten egg' odor is important to your safety in
identifying a propane leak, should one occur. If you purchased propane in
one of those areas during the time period stated above and have not
refilled, we recommend that you take your tank to the nearest propane dealer
and have it topped off to make sure it contains sufficient odor. If you
require additional information, please call this toll-free number during
normal working hours (Central Standard Time): 1-800-874-3554."
